How to apply:You can fill out our Online Application Form.
Please Note:
* Grant Applications will only be accepted during each of the two separate "Application Periods". Application periods are defined as the month of June and the month of October.
* All blanks are required.
* Dates need to be numbers only, i.e. 10-7-10 (no alpha).
* Phone numbers need to be in
(XXX) XXX-XXXX format.

You can use Adobe Reader to print out your Application.
Then fill it out, scan it, and email it in.
